Overview
It has been stated that the world was unified many years ago, most attributing the established date as "0 U(nification)". The "United Kingdom" was first a small, human kingdom that was fractured after a civil war thanks to the death of their King Uther in the year 564 BU. It wasn't until the late king's son, one Arthur, received a magical blade said to be a gift from the Lady of the Lake quickly crushed the rebellion was peace known. The Pendragon Civil War is thought to have only last 2 years, but remnants of rebels survived long after that. After that, Arthur focused on uniting their large island-country.
After the country of Albion was established, named such by King Arthur after the city he was borne in, he had settle for living the rest of his life in peace. He was content with watching over his kingdom until they day that he, too, would perish. Yet, Arthur's half-sister, one Morgana, had deployed her devious plot. One night many moons ago did the Morgana employ a powerful hex to bed her half-brother and create a child by the name of Mordred. Morderd was everything one would want in a knight, being strong, swift, and, most importantly, loyal. No scholar knows if this was perhaps one of Morgana's many spells she had played on Mordred in order to make her a better knight than most of if that was simply the Pendragon lineage and harsh training she had undertook. Regardless, one day Mordred revealed her lineage and proclaimed herself the daughter of Arthur. King Arthur denied all of it and simply referred to Mordred as some hellspawn crafted by his half-sister.
On that day did Mordred turn her back on the Albion Kingdom and held up in a long-forgotten fortress on the eastern islands. Mordred's power and tale of being a castaway child had united the rebels under her banner. "The Black Hands" is what they were called. Many believe that Mordred's might and Morgana's magic would have spelled doom for the kingdom if it wasn't for Merlin's, an accomplished wizard of the court, plan to directly assault their capital. Many attribute the battle that took place at the Black Fortress as one between titans. Monsters and men took both sides, dragons of blue and white striking at blackened dragons, giants of northern wastes swatting at majestic griffons of the mountains-- all of it accumulated into the final battle between Arthur and Mordred.
The duel was almost Arthur's, yet a timely intervention of Morgana, who Arthur slew with a single stroke of his blade, gave Mordred the opportunity to drive her blade deep into Arthur's side. This was almost assuredly a fatal blow for Arthur, but Mordred had taken many of her own. She had saw Arthur's Knights of the Round Table draw ever closer as they fought their way past her own elite knights the Red Hands. Knowing that she had lost, she had withdrawn and fled on a ship. Morgana's body was never recovered by the kingdom of Albion and the islands she had supposedly died on were forever tainted by the forces of chaos.
Arthur needed many years to heal, but when he could move without pain in his chest once again he had ensure to personally lead his armies into the unknown seas. There had been little travel between the neighboring-by-sea countries as the civil wars had kept the Albion Kingdom busy for many years. Arthur, which many assumed was blinded by anger, had now made it his life mission to hunt down wherever Mordred may be and kill or subjugate those who were in his way. So began the worldwide crusade for Mordred. Few powers managed to avoid King Arthur's wrath, mainly the High Elves of Ulthuan and the Dwarven Empire of the Mountains. These two kingdoms were thought of to be too powerful or perhaps Arthur knew they would never side with Mordred-- although in our times we do know that certain elements, Dark Elves and Chaos Dwarfs, had assisted Mordred for their own reasons.
Then there were nations that were too far from the path that Mordred had travel. The empire of Cathay and kingdom of Nippon are too examples of this as they were too far in the east to be considered where Mordred would have gone. Naturally speaking, Arthur spent many years accomplishing his tasks. The Holy Empire's war was one that lasted nearly 45 years as territory was lost and gained at rapid rates. The attrition-based war with Kislev last over 100 years by most estimates. This is not including the various rogue factions rising up every once in awhile such as the great Beastmen Herd of Khazruin or the Green Tide of Igzac the Chosen. Yet, after this central continent was subjugated, Arthur made his way east to the "New World" as many had called it.
In the New World, Arthur had carve a new kingdom one that was referred to as Colombia. It was here that many humans of various nations and even a multitude of monster-races gathered to exploit these new lands. Upon the final piece of land being claimed by Arthur, the year hit 0. It was north that Arthur still had to venture to-- seeing as the south was filled with lizardmen, gold, and other beasts that he was so sure Mordred did not travel to. The frozen wasteland north of Colombia is Kanata. Mordred's new Black Fortress was believed to be on the very northern edge of it, but she made sure that Arthur suffered every step of the way to her.
Many of the Knights of the Round Table had perished and some have even turned. Arthur's first advance into Mordred's land revealed Lancelot's true colors as he attempted to kill the king while the dark forces of Mordred marched upon them, for example. Arthur's own wife had perished many years ago by what many consider a plot by Mordred involving the slow poisoning of him. Yet, Arthur has not once shed a tear of his fallen comrades or family. Instead, he continued to march.
No one knows exactly what happened during the Final Battle, but it is believed that after Mordred was slain Arthur shed his mortal form and joined the gods in the heavens. Others say that they both slew each other, but no one was brave enough to enter the arena they dueled in. Some, although quite few, believe Mordred had killed Arthur and once again fled. Regardless, without their Dragon King to lead them and end rebellions with a single slash, the empire he had built started to crumble. Many once-subjugated empires had slowly sprung back up as barons claimed they require more independence to deal with their own various issues.
There are many other notes in history aside from Arthur such as the splitting of the High Elves who, after a dispute over the next Phoenix King, ended with much blood spilt. Some say Mordred's forces even played a part within the "Elf Inner Turmoil" as the elven scholars called it, having agreed to assist the Dark Elves if they would assist her in return. Then the Great, Grand Disasters that, within a matter of years, saw many dwarven holds destroyed by natural causes or hordes of the Greenskins or Skaven.

Nations
I am far too lazy to create many different nations, so just accept WFRP4e ones as mostly canon to this setting. Although there would be slight alterations. Just like how I'm too lazy to create a buncha gods.
Kanata
Kanata is simply Canada, but with more snow, CHAOS, and Dark Elves. After Mordred's apparent defeat, they moved in and claimed it all. It has short months where farming is possible, but for the most part the Dark Elves focus on raiding and pillaging everyone else. Trade between Kanata and the Free State is frequent, specially for those in the criminal underground world.
The Free State of Colombia
Colombia is, simply put, the US. It has various of cultures and races that intermingle with it with the Bretons still holding plenty of governing power here. If I were to run a campaign, it would be set here since anyone could be anything they wanted to be.
